This form is for anyone wanting to reserve their seat/placement in an EMS Education course offered by Firestorm Training & Consulting, LLC.

These EMS courses require a 10% deposit to reserve your seat. All deposits are NON-REFUNDABLE. Payments are made through Venmo, CashApp, personal check, cashier's check, or Debit/Credit card (3% processing fee).

📚 Course Selection

📋 Important Course Information:

  • Course Schedule: The EMT-Basic course runs in the Spring semester (January-June), and the Advanced EMT (AEMT) course runs in the Fall semester (August-December). Only courses that are currently open for registration will be displayed below. Courses will appear on this page when registration opens, typically closer to the start of that semester.
  • All EMT-Basic courses include American Heart Association BLS (CPR/AED) certification
  • Advanced EMT courses include both BLS and ACLS certifications
  • AEMT students must have EMT-Basic certification before clinical rotations begin
  • Attendance is mandatory per Texas Department of State Health Services requirements
  • EMT-Basic: TWO mandatory Saturday classes
  • AEMT: THREE mandatory Saturday classes
  • All payments are non-refundable

Once your registration is received, you will be contacted by email to set up a payment method for the 10% down-payment to reserve your seat. Your registration will not be processed until payment is received.

📋 Payment Agreement

I agree to pay the listed course fee or any remaining balance by the due date. Continuance in the course, course completion certificates, and any other associated certifications will not be issued until payment is received in full.

You understand that you are required to remit payment as described above. You understand that all payments are final and non-refundable. You also agree to adhere to all rules and policies while attending training conducted by Firestorm Training & Consulting, LLC. Any complaints or concerns shall be generated in writing and sent via email for review.
